Description of Service:
By filling in this waiver, I, acknowledge that I am receiving a piece of jewellery from Eternal Bond Permanent Jewellery and availing of their welding service to create a permanent jewellery item.
Assumption of Risk:
I understand that jewellery, like any delicate item, may be subject to wear and tear over time. While Eternal Bond Permanent Jewellery takes all necessary precautions to ensure the quality and durability of the welding service, I acknowledge that there is inherent risk associated with wearing any jewellery item, including the possibility of breakage, loss, or damage. I also acknowledge that all jewellery chains have been intentionally designed with a weak point for safety reasons, so that in the event of excessive force, the chain will break rather than cause injury. This is a precautionary measure for my safety, and I accept this risk by wearing the jewellery.
Allergies:
I acknowledge the risk of allergic reactions to metals in permanent jewellery and release Eternal Bond Permanent Jewellery from liability for any reactions I may experience. I understand that even hypoallergenic metals can cause reactions and accept this risk by signing below.
Breakages/Damage to Jewellery:
I acknowledge that Eternal Bond Permanent Jewellery provides a complimentary welding service for the jewellery chain purchased from them, if it breaks at the original welded point within 28 days of the initial welding service. I must contact Eternal Bond Permanent Jewellery and present the jewellery item for repair during this 28-day period. In such cases, the welding service will be provided free of charge. I acknowledge that if the chain has been broken, then a replacement or repair cost may be charged.
This breakage policy applies only to jewellery chains that have been purchased from Eternal Bond Permanent Jewellery.
Care and Maintenance:
I understand that proper care and maintenance of the jewellery chain are crucial for its longevity. Eternal Bond Permanent Jewellery will provide me with guidelines for maintaining the jewellery item, and I agree to follow those guidelines to the best of my ability.
Liability Waiver:
In consideration of the services provided by Eternal Bond Permanent Jewellery, I hereby release and discharge Eternal Bond Permanent Jewellery, its employees, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any damage, loss, or injury that may occur while wearing the jewellery chain purchased fromEternal Bond Permanent Jewellery or during any welding service provided by them.
Photography and Marketing:
I give Eternal Bond Permanent Jewellery permission to photograph my jewellery and procedure for their own marketing and promotional purposes.
Legal Capacity:
I confirm that I am of legal age and have the legal capacity to enter into this waiver. I have read this waiver carefully, understand its contents, and voluntarily agree to its terms.
